Has anyone used a Zenmuse gimbal and configured it as a handheld mount? Does it have to be attached to a Phantom to operate?

This has been covered before.

An H3-2D or 3D needs to be connected to a Naza FC since it receives feedback from the accelerometers and gyros within the Naza AS WELL AS it's own internal GCU.

Get yourself an Alexmos and save yourself the trouble.
