The Nuzzle pet tracker looks like a potential good fit for a Phantom GPS tracker.

Here are the relevant specs:

• Weighs about 1 oz
• Collar can be submerged in 10 feet of water for up to 30 minutes
• 2 removable/rechargeable batteries
• Battery lasts up to 5 days
• Location accuracy of up to 10 feet
• Location updates as quickly as 1 minute
• Dual-band 3G cell connectivity
• Bluetooth smart-enabled
• Works in US only
• Free iOS/Android app
• $189 and no monthly fee

The size of the GPS unit is not mentioned, but it appears to be no larger than other commonly used Phantom GPS trackers. It's available for pre-order now and is supposed to start shipping next month (November 2016).
