How come sometimes you lose signal and RTH is initiated but other times I get " Disconnected" message and seem to have no control and RTH is not initiated? If it "disconnects" shouldn't it return to home?

RTH is only initiated if the remote controller disconnects from the Phantom. It's not initiated when the downlink (the live video) disconnects.

A disconnect can often be confused with no signal. The live video feed can be disconnected while there is a control signal still present. The drone will not RTH while there is any control signal still available.
