Vision - Camera fell off during flight

The camera just came off the Vision and dangled by the wire during mid-flight. Has this happened to anyone else? It was maybe my 7th flight.

I can't figure out how to reconnect the camera.

Looks like you are missing some screws. There is 3 bags of screws in the box. 2 of them must fit.
