Gimbal Lock - can't get it back on

A week or two ago someone had posted a photo of how it goes on. I can't figure it out and I didn't take a photo of my own - now I can't find the photo. How do you get the **** thing back on there?

Exactly what I needed. I even saved them to my dropbox so I can reference them in the field!
