Flight Log and Checklist

I know this has been discussed in multiple threads, but I don't see any of them near the top anymore.
So, here's something I put together for my own use. It's a compilation from many sources.
I'm sure it's not perfect, and I know some of you think checklists are overkill. But it might be useful for someone.
It's designed to be printed 2-sided, on one sheet of paper.
Flight record(s) and notes on one side, and checklists on the other.
Feel free to use/comment/bash all you want.

Very nice but...how can it be converted into a PDF form that can be a fillable form so it can be filled out on the screen then saved and archived?

I made it in Word, which is what I use to fill it out. I might be able to make a PDF form from that. If I find some time, I'll look into it.

One other pre-flight step I take is to reboot my iPad so nothing else is running. I know you can close all apps but some like facebook still seem to stay in the background and popup updates and such.
