More likely issues ramp-ing up the vendor(s). i.e. Growing Pains.
The stories you read are anecdotal reported by the general population which statistically lack the experience to make such root-cause determinations.
If you have never worked in high-volume manufacturing you may not appreciate what it takes for just that alone.
Now add the EXPLOSIVE growth and demand for these parts and products and it's easy to get into a situation where you can't keep up.
Having said that there will always be escaping defects which result in field failures.
