Can someone help me with these errors?

You most likely either have a bad compass calibration or you took off near some type of magnetic metal object. If you did not do the latter, then try calibrating your compass outside in an area where there are no metal objects nearby (including your watch, phone, etc.).

Not sure I am getting so many of these. If it something noobish sorry.
Where did you take off?
Was it on a reinforced concrete slab?
Where/when did you calibrate the compass?

The indications from the flight record are as msinger said above.
The Phantom has had a problem with conflicting data from the compass and GPS and has switched to atti mode as a resuly.
This may have made the Phantom a little difficult to control if you haven't practised in atti.
Also it's not related but taking off with a battery at 88% isn't a good idea.
Always have a fully charged battery when you fly.

What happens when you lift from the grass?

Any idea on the yaw error?
Same deal. Those errors were most likely caused by a bad compass calibration or taking off near a metal object.

Hell, I never take my car out of the garage if I have less than a full tank!
