You can already do this in Mission Hub. Just click on a waypoint, then click the "insert" button. A new waypoint is added between the one you selected and the next one.

My theories: motor failure (or other mechanical failure on one of the motors/props) or bird strike (though I don't see a bird in the video).

My iPad is on OS X 9.2. Litchi on the iPad is version 1.2.1. I don't recall the firmware on the Phantom when it was lost, but it is the latest one as of mid-January.
